Talent ID Camp Release of Liability & Content Ownership Waiver

I hereby assume all of the risks of participating in this AOCVB Talent ID Camp, including by way of example and not limitation, any risks that may arise from negligence or carelessness on the part of the person or entities being released, from dangerous or defective equipment or property owned, maintained or controlled by them because of their possible liability without fault or which result from any of the other risks described below.

I hereby acknowledge that an inherit risk of exposure to COVID-19 exists in any public place where people are present. By participating in the clinic, I voluntarily assume all risks related to the exposure of COVID-19 and agree not to hold Total Sports, LLC, its partners, the host facility, or any of their affiliates, directors, officers, employees, agent, contractors, or volunteers liable for any illness or injury.

I certify that I am physically fit and have not been advised to not participate by a qualified medical professional. I certify that there are no health-related reasons or problems, which preclude my participation in this AOCVB Talent ID Camp. I acknowledge that this Waiver will be used by the organizers of the AOCVB Talent ID Camp in which I may participate and that it will govern my actions and responsibilities at the AOCVB Talent ID Camp.

In consideration of my application and permitting me to participate in this AOCVB Talent ID Camp, I hereby take action for myself as follows:

1. I WAIVE, RELEASE, AND DISCHARGE Total Sports, LLC, its partners, the host facility,  or any of their affiliates, directors, officers, employees, agent, contractors, or volunteers liable from any and all liability of any kind or nature, including but not limited to, liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from this AOCVB Talent ID Camp.
2. I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE Total Sports, LLC, its partners, the host facility, or any of their affiliates, directors, officers, employees, agent, contractors, or volunteers for any and all liabilities or claims made as a result of participation in this AOCVB Talent ID Camp, whether caused by negligence or otherwise.

I acknowledge that this AOCVB Talent ID Camp may carry with it potential for death, serious injury, and personal loss. The risks may include, but are not limited to, dehydration, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, slips and falls, contact or collision with other participants, staff, equipment or other facilities or actions of other people including, but not limited to, participants, volunteers, spectators, coaches.

I hereby consent to and authorize the use and reproduction by Total Sports, LLC, its partners, the host facility  and their sponsors of any and all photography and/or videography that has been taken of me for any purpose, without compensation to me. All digital image and/or video files are wholly owned by Total Sports, LLC  and its sponsors, who reserve the right to use these images and/or video for publications on their websites and in marketing materials.
